If you would like to enroll in the New York State Donate Life Registry, please provide the information requested below. Within a few weeks you will receive a confirmation letter from the Commissioner of the New York State Health Department.

Enrolling in the New York State Donate Life Registry is one of the most important things you can do. By becoming an organ, eye and tissue donor, you have the capability to save and enhance up to fifty lives.

In addition, your enrollment lets your family and loved-ones know of your intentions and eases the decision and consent process after your passing.
