What is the Organ and Tissue Donor Registry?
The registry is an official listing of all New York State residents who pledge to give the gift of life through organ and tissue donation in the event of death.
Why do we need a registry?
Family consent is required for organ and tissue donation. Many families do not consent to donation because they are unaware of their loved one’s wishes. The New York State Organ and Tissue Donor Registry will help to ensure that an individual’s wishes are honored by family members and health care providers.
What happens when I enroll?
Enrollment in the donor registry declares your intent to donate your organs and tissues after your death. In the event of your death, UNYTS will inform your family of your decision. Your family will still have to give consent, so it is important to tell them about your wishes to become a donor.
If I enroll in the registry, should I still sign a donor card or the back of my driver license?
Yes. This will help to ensure that your wishes will be carried out.
Can I change my mind after I enroll?
Yes. If you decide you do not want to become an organ or tissue donor and you want your name removed from the Organ and Tissue Donor Registry, write to:

How can I enroll in the registry?
There are several ways you can enroll in the donor registry:
- You can submit a request to have your name added to the donor registry via this Web site - just click here.
- When you apply for or renew your driver license for the first time, you can indicate your intention to donate by checking a box.
- You can request a brochure and enrollment form from UNYTS - just click here.
- You can request an enrollment form from the New York State Department of Health Web site at www.health.state.ny.us.
When you enroll in the donor registry you will receive a confirmation letter from the New York State Health Commissioner. Please keep in mind that enrolling in this registry is only a way to express your intent to donate--it does not guarantee that your wishes will be carried out. Your next-of-kin must give consent for donation, so it is vital that you share your wishes with your family.
